Online Consultation
- You do not need to register for this service.
- You will be directed to a form that you will need to complete in relation to your problem or query.
- Our Admin Team will review your query and identify the correct pathway for you.
- Please click the button below to be taken to our online consultation facility.
Telephone
- You can contact us on 01572 722 621
- Should you be advised that you are a priority patient you will be provided an alternative number to contact the surgery.
- You will be spoken to by a trained member of our Patient Services Team.
- Please answer their questions as fully as possible to identify the best pathway for you.
- Please understand that we will do our best to support you, however this may mean identifying a different pathway than the one you have asked for.
- We ask that you please be respectful when interacting with our staff.
Online Services

- If you do not have an online account, you will first need to register, using the online services.
- Once logged into your account, you will be able to view and book all available appointments.
